Imaging response evaluation after neoadjuvant treatment in soft tissue sarcomas: Where do we stand?

• Neoadjuvant chemo-radiotherapy is increasingly used to treat soft tissue sarcomas. The radiology-pathology correlation of treatment-related changes not yet settled Standardization in reports critical evaluate correlation. Choi criteria better reflect tumour response compared RECIST criteria. Radiomics emerging as a powerful tool predict response. Soft sarcomas (STS) represent broad family rare tumours for which surgery with radiotherapy represents first-line treatment. Recently, neoadjuvant has been high-risk patients an effort reduce surgical morbidity and improve clinical outcomes. An adequate understanding the efficacy therapies would optimise patient care, allowing tailored approach. Although evaluation solid (RECIST) most common imaging method assess response, functional molecular (DWI, DCE-MRI 18 F-FDG-PET) seem outperform it discrimination between responders non-responders. Moreover, radiologic-pathology remains poorly understood. In this review, we provide overview assessment STS undergoing treatment, including conventional (CT, MRI, PET) advanced analysis. Future directions will be presented shed light on potential advances pre-surgical assessments that have implications sarcoma patients.
